英訳をお願いします(レストランへの予約)

ハワイのレストランへ、予約の件でメールを送りたいです。
団体なのでオンライン予約ではなくメールでやり取りしています。
予約だけはすでにしています。

送りたい内容
・人数が確定した。合計20名
・メニューを決めた（宴会セットメニューB)

一応自分で考えてみたのですが。
Number of party : 20adults
We'll take the Party set menu B($40)

差出人（代表者）の住所氏名電話番号も忘れずに、（いたずらもあるので店としては警戒しているはず）

I make a reservation, so please take care.
The number of people was fixed. Total 20 people→又は20adults
The menu has been decided. Banquet set menu B($40)→またはthe Party set menu B($40)
差出人（代表者）の住所、氏名、電話番号（The address of the representative, the name and the telephone number）
