英文メール、教えてください。。。

2年前まで子供がアメリカのミドルスクールへ通っていました。
受験に伴い、成績表と在籍証明書を請求せねばなりません。。。取り急ぎ、メールにて問い合わせさせていただこうと思っています。どのような文面で請求すれば良いでしょうか。。。どうぞお助けください。宜しくお願いします。

My son/daughter I.Suzuki was enrolled in your middle school from 2009 to 2012 in classes of Mr. George Smith and Miss Mary Jones.

In conjunction with his/her entrance examination he/she has to present two documents:(1) certification that he/she was enrolled at your school 2009-2012, and (2) his/her grade report during the period.

Could you kindly send these documents to the following address, and tell me how much I should remit for the documents and the postage?
