もう必要なくなっているのに抗生物質（の服用）を続けること、

using a broad spectrum antibiotic when it is not required or continuing the broad spectrum antibiotic unnecessarily after the sensitivity results are received,
必要がないのに幅広い種類の抗生物質を使ったり、過敏反応が認められた後も幅広い種類の抗生物質を使い続けたりすること

using the wrong antibiotic or prescribing the wrong dose,
間違った抗生物質を使ったり、間違った服用方法を指定すること、

or continuing the prophylactic antibiotic after it is no longer recommended.
あるいは、推奨されなかなった後も（推奨されている期間を超えて）予防のための抗生物質（の服用）を続けること

区切りは少し難しいですが、難しいところで迷わないようにコンマが打ってありますので、それを頼りに読んでいけば大丈夫。
