この文は仮定法過去なので下ほうは間違えなのですか？

間違えの答　　Suppose a serious crisis had arisen
→Suppose a serious crisis arose, なら正解です。
ただし、aroseというのは堅苦しい言い方です。

この文は仮定法過去なので下ほうは間違えなのですか？
→そうです。仮定法過去完了（過去のことを仮定）にしたのが間違いです。

この文でhis patientは複数形なので下のほうではないかと思ったのですが、間違えですか？
take it easyが答です。
この場合、insisted that ... となっています。このinsistが、要求、願望（つまりこうあってほしいという仮定法的な意味を含む動詞の意味）のときは、that節の中はshouldを使うか、あるいは、そのshouldをとったまま原形動詞を使うかのどちらかになります。
I insist that he keep early hours. 彼にぜひ早起きをしてもらいたい。
I insist that he should keep early hours.
（この２つの例文はジーニアス英和大辞典より引用）
このようになります。he keepsは間違いです。
ぜひ、辞書で、insist, propose, suggest, orderの4語を調べてみてください。他にもあります（demand, advise, recommend, requestなど）が、この４つは大切です。

これは文法的には正しいのですが、現代英語としてはほとんどありえないものです。日本語で言えば古語で「もし重大な危機が起こらば」と言う感じでしょう。なぜなら、

１）そもそも現代英語では仮定法自体が衰退していますので、一人称や三人称の単数主語でwereを使うことは口語ではまれです。文章では推奨されますが。
2)仮定を表す節において、ifを使う代わりに主語と動詞を倒置するのはかなり古い表現で、現代英語では文章でもほとんど使いません。
3)(be) to doは不定詞の未来形で、～するはず、とか、～するべき、と言う意味で、状況によってはいまでも使いますが、If a serious crisis were to arise　であってもかなり古い感じです。

従って、仮定法にこだわるとしても、If there arose a serious crisis, あたりが普通でしょう（これも口語ではまずないですけど）。


The doctor insisted that his patient ( ) for two months.
これは仮定法未完了で、イギリス英語だとshould take、アメリカ英語だとtaleになります。後者はshouldの省略ともいわれますが、英語としてはむしろこちらの方が本来の形です。まともな文章英語であれば、takesとかtookにはなりません。

Oxford大学のMicheal Swarn曰く、suggestはinsistと同じでthat節を導いたときにthat節の動詞がアメリカ英語では原形で、英国英語ではshould＋原形を取ることでsubjenctive(仮定法）を表現することができるとしていますが通常動詞を取っても何ら問題は無いとも言っています。

a) Ordinary persent and past tenses can be used in British English.
Her uncle suggests that she GETS a job in a bank.
He suggested that she GOT a job in a bank.

b) In a formal style, subjenctive are possible, especially in American English.
He suggests that she GET a job in a bank.
He suggested that she GET a job in a bank.

c) In British English, SHOULD + INFINITIVE is common. (This is rare in AE)
He suggests that she should get a job in a bank.
He suggested that she should get a job in a bank.

ですから英国英語ならば

take it easy
takes it easy

はどちらも間違いでない。また、アメリカ英語もinsistやsuggestの後にくるthat節は原形動詞を持つこともできるし通常の動詞を使っても良いはずですよ。
