受動的な含みは無いと思います。
「結局」のようなニュアンスでしょう。

sufferingの意味は「苦しんだ」ということ。
厳密に言うと、周りの人が異常に気づいて病院送りにした可能性も考えられます。
本人は「訴え」ていないかもしれません。
