下記の英文の関係代名詞 ”who” は、文脈からアメリカの兵士のことを指していると思われますが、
英文の構造から考えると。”Afghan villagers”を指しているように思われます。

who は Afgan villagers を指しています。

'cut'と'kept'はそれぞれ並列で過去分詞です。

'Afgan villagers who had their fingers cut off and (who had their fingers) kept as trophies'

ということで「指を切り落とされて、戦利品としてもっていかれた村人たち」ということです。

have+O+p.p. の形はふつうは「Oを～される」と訳されますが、直訳すると「指が切り落とされて、戦利品として取っておかれる状態をもった」ということです。
