文章の右へ行けばいくほど、英文は新情報=耳目を傾ける他者に伝えたい語句情報になります。

通常の書き方ではこうなります:

because they use [what brain tissue they do have left] so efficiently

[what brain tissue they do have left]は「残された脳の組織」という意味で、"use"の目的語ということになります。"do"は強調のために使われています。

なぜ倒置されているのかと言いますと、目的語が長すぎるのが一つの理由でしょう、
