日本の役所に出す書類（ご質問のような内容の書類も含め）、は、最上段に日付、次の段に宛先と宛名、そして更に一段下がって差出人（訴状であれば原告の代理人）を書き、それから本文が始まります。

　そして本文の最後に「以上」を書いてそれで終わりの事が殆どです。　従って英文書類の様式と違うので Respectfully submitted のような締めの言葉はありません。

従ってご質問のような英文の書類の Respectfully submitted を日本語に直すのであれば、私ならあっさりと「以上」で済ますかもしれませんし、あるいは英語の意味を尊重して「謹んで提出致します。」とするかも知れません。

署名の下の By: NAME のほうは 「氏名」だけでよいと思います。　英文でも NAME　だけのことがよくあります。

Thanks - what that a super reply.

For my complaint I'm using a traditional Japanese cover sheet with the date, signature and everything you mentioned. However... because this is 101 pages long, I've used the same ending style as the English.

Now you have me thinking if I should be Japanese, and just delete the ending. :o)

頼み、文句、苦情、訴状 →　依頼状、文句／苦情申立書、訴状　と考えると、

「謹んで提出いたします」。

１６行目の前に、XXX裁判所とある場合は、「御中」でしょうか。

I like this ! I always like your translations. They are always very literal. Nice !

This is good because I don't think it's a custom to write such at the end of a document however I don't care. It makes more sense to sign at the end rather than the beginning, so that's what I'm going to do.

I do wonder though. I didn't use ます形 in the document even once. I used 辞書形... so I'm wondering if I should end with ます　

eg: 食べます→食べる　　　します→する　　など。。。
