仮定法過去完了と思われます。
if I had been a 240-pound Ken Miles ではないのでしょうか？
教科書的にはこの方が正しく思うのですが、別の意味があるのでしょうか？

(9) 平行構造：'d have...'d have
形式貼らない話し言葉では、if 節が過去のことを指すとき、'd have を用いることがある。
この用法は正用法ではないと考えらえれることが多いが、教養のある人たちの話し言葉にも頻繁に見られるものである。
書き言葉では用いられないのが普通である。

If I'd have known, I'd have told you.
もし知っていたなら、君に話したのに。

It would have been funny if she'd have recognised him.
もし彼女に彼が誰だか分かったとしたら、おかしなことになっただろうね。

縮約形の 'd have の代わりに、完全形が強調のために用いられたり、また否定文で用いられることも時々見かける。
had も would も両方とも現われる。

I didn't know. But if I had've known...

M. Swan氏の文法書に口語でよく起きると以下のように説明があります。

In informal spoken English, if-clauses referring to the past are sometimes constructed with " ‘d have". This is frequently considered incorrect, but happens quite often in educated people's speech. It is not normally written.
　If I’d have known, I’d have told you.
　It would have been funny if she’d have recognized him.
(Practical English Usage, §262.2)

I'd 've

はI would haveの短縮形で I would’ve; I'd have; I wouldaとも言う。質問文のピリオドの使いかたがおかしいので調べてみたら

"These cars are not made for large men. Wouldn't have been the same thing if I'd have been a 240-pound Ken Miles, barely, barely getting into those cars," Bale said in a press conference during the Toronto Film Festival.

という使い方をされてたようです。～とBaleさんが言ったと引用符を入れてるので文法は校正されずに記事になってる。
文法をただすなら貴兄の指摘のようにhad beenが正しい。

明らかに if I had been a 240-pound Ken Miles の間違いです。

おそらく If I'd been a 240-pound Ken Miles とすべきところを I'd のあとに勢い余って 've も入れてしまったとしか考えられません。
