the first twelve hoursの意味は、直訳したら最初の12時間という訳になると思うのですが、解説には最初のとは書かれてませんでした。
他に訳し方があるのでしょうか

解説ではどう訳されていたのですか？　その訳文に「the first twelve hours」の意味が過不足なく含まれていれば、必ずしも「最初の」という３文字を使わなくてもいいと思いますが。

この場合は日本語では「出産後最初の12時間」とは言いませんね。

「出産後１２時間」と表現するのが普通です。

「分娩（生誕）直後の１２時間」としても日本語では意味は同じような感じになると思います。
