お誕生日がおなじ日ならば、

Today’s his and my birthday.
お誕生日がちがう日なら、
His and my birthdays are two days apart.

フレーズだけでは birthday の単複はきめられません。
文章の内容できめます。

〇〇は代名詞の所有格（yours, his, herなど) か名前であれば Mai's, Joe's のように ['s]が付きます。

そして複数の人の誕生日なので birthdays になります。

(例）
His & My Birthdays
Ken's & My Birthdays.
