1個不良が出て、不良の内容を聞くときに、「その1個は何個中の1個ですか？」と訊きたいのですが、英語ではどういうのでしょうか。5個入荷したうちの1個、だとか、10個入荷したうちの1個、というのを知りたいのですが・・・。

何個、何人中のいくつは

8 out of 10 believe... 十人中八人は～を信じている

We found one item damaged(defective) out of 10.
十個中一個が不良でした

How many out of ten did you find damaged?
１０個中何個が不良でしたか

何個中の一個ですか

Out of how many did you find the one damaged?
しかし、この言い方は実用的ではありません。
実際の場面では総数が当然わかっているはずですから、総数をたずねる必要はないはずですから

報告書では

One out of ten wss reportedly damaged. となりますから

いろいろの言い方がありましょうが、

Would you let me know what percentage of rejects you have?
Would you let me know what percentage of defective parts you have?

さらにしつこく書くとすれば

Would you let me know the number of rejects, n, and the percent defective, n/N?

Could you tell me, out of how many you found the non-working item?

１行目は前置きで「１個不良品があったと伺いました」

　それから本題に入り「その不良品は何個中の一個か、教えていただけませんでしょうか」

　としました。

　何でしたら説明のため We would like to know the percentage of failed items in our shipment 「こちらから出荷した品目の、何パーセントが不良だったか知りたいものですから」とお付け加えになるのも一案かと思います。
