疑問詞do you think SVと教わったのですが、

what do you think her name is?
はisが最後に来ているのに対して、
who do you think is the smartest guy?
はisが最初に来ているのでしょうか？
who do you think the smartest guy?
or who do you think the smartest guy is?
ですか？
こんがらがりました。

原則は、じろー2525さんが学ばれた語順の通りです。

学習段階では、この語順を守るようにしておけば大丈夫です。心配いりません。

ただ、細かく言えば、Who do you think the smartest guy is? は「smartest guyは誰か」と、補語について尋ねており、
お気づきのWho do you think is the smartest guy? の文は「誰がsmartest guyか」と、主語を尋ねています。
ですから、この場合は両方の文が可能です。

もうひとつ、補語についての疑問詞疑問文でも、その補語が長い場合、SVが逆転することがあります。「倒置」です。
-Who do you think is the intended audience for these plays?
ここではSであるthe intended audience for these playsが長いので、isと倒置され、文尾に追いやられています。

以上の二つの例は、そう頻出するものではありませんが、一応知っておくと便利かと思います。

はisが最初に来ているのでしょうか？
とありますが、二つ目の文は、
Who do you think the smartest guy is? です。
答は、I think Taro is [the smartest guy].
こんがらからないようにするには、まず、
疑問詞＋do you think +s+v を確認し、think の後がs+v になるという事は、
そこは平叙文の形ということなので、be 動詞はSの後ろ、一般動詞も当然Sの
後ろに来て、その際、do,does,did などを使った疑問の形にしてはダメという
事です。
Who is the boy?
⇒ Who do you think the boy is? ; think の後、is the boy ではない ;ｓ＋ｖの形
～I think he is Taro.
Who does Mary love?
⇒ Who do you think Mary loves? ; think の後、does Mary love ではない; s+v の形
　 ～I think she loves Taro.
何度かはこんがらかるでしょうが、think の後ろはｓ＋ｖを思い出せば大丈夫です。

who do you think is the smartest guy? -> whoは主語。
who do you think the smartest guy is? -> whoは主語ではない（補語？）。
whoを主語にしてもしなくてもこの場合は意味は通るので、どちらも正解でしょう。
一つ目の文は、誰が一番賢いの？、二つ目の文は、一番賢いのは誰？

Do you thinkを取ってみましょう。

そして答え方を見てみましょう。

What (do you think) her name is？　What is her name?　その答えは、Her name is Jane.（Jane is her name という答え方は通常しない）

Who (do you think) is the smartest guy? Who is the smartest guy?　Steve is the smartest guy. （The smartest guy is Steveという答え方は通常しない）

ネイティブにまだ確認していませんが、私の個人的な考えでは、上の例からわかるように、答えの文型に合わせて質問ができているのだと思いますよ。
