学校などから発行される修了証明書を英語で書くとどうなるのですか？

のような感じの証明書の場合英語だとどうなるのですか？よろしくお願いします！！

　日本の証明書と違い、色々な種類があるようです。

私がもらった短期の語学コースの修了書のケースを例にあげときます。

(1)　　　 　　　 名前
　 has attended a course
in
English Language
with Brtish Histry, Society & Culture

at
　　　　学校名
　　　　　期間
　　　　　授業数
　　　　　Level of Course

presented on:

Signed on beharf of 学校名

　　　　　　Signature (職名）

（2）　　　　　Certification

the undersigned declare that
　　 名前
has attended the course Communicative English

The course took place 　期間　and comprised the
following subjects:
training the speaking skills;
training the listening skills;
expanding vocabulary;
understanding Dutch and English culture.

　　場所, 日付け

(3) CERTIFIES THAT 名前

　日本みたいに「記」というのはないんじゃないかなあ。
