a bookとthe nameってどっちも名詞ですよね？
こんな形取れるんですか？

はい、取れます。

この形の大体の説明としては、まずHe has written a book whose name I have forgotten.から始めます。
whose nameがhave forgottenの目的語です。ただ、本に対してwhoseを使うという点に違和感があります。
そこで：
1. He has written a book the name of which I have forgotten.
2. He has written a book of which the name I have forgotten.
という形が可能になります。
いずれもI have forgotten the name of the bookという文のbookを関係代名詞にして文頭に出す際、修飾関係を構成しているthe nameとofが一緒に移動したものです。
しかし、2.は既に使われていません。学習英語では1.が使われますが、実際はこれも古くさいとされています。

ですから英作文や会話でこの内容を示す時には、He has written a book, but I have forgotten the title/name. のように表現した方がいいようです。
