but は、only に似た意味を持っています。｛～は、～でしかない」というような意味だと思います。

(2) which は、その前の osseointegration を指しているというよりも、
a foreign body response to the implant
の全体を指しています。もっと言えば、implant を飛び越えて "a foreign body response" だけにかかっていると言った方がわかりやすいかもしれません。

そのように考えれば、

(5) which is a chronic inflammatory response

というわけで、
