(1) 間違い：　questions about THAT he would ever the same after such an accident?

"about that S V" なんていう英文は間違いです。"about the reason why S V" とか "about the fact that S V" なら正しいです。文法用語を並べて理屈を言うのではなく、そのまま覚えればいいのです。

(2) 間違い：　questions about WHAT he would ever the same after such an accident?

(3) 正しい：　questions about WHETHER OR NOT he would ever be the same after such an accident?

(4) questions about WHY he would ever be the same after such an accident?

なぜ名詞節になるかなんて言わず、これは名詞節になるんだとそのまま覚えればいいのです。
