1. She has never been seen to cry (by us).

2. It has been decided (by them) to hold the meeting on Monday.
It has been decided (by them) that the meeting is [will be] held (by them) on Monday.
2例目の能動態は
They have decided that they hold [will hold] the meeting on Monday.
時制は予定の確実度が高ければ現在形の場合もあります。(hold)

3. A housewife is said to have written this great novel.
This great novel is said to have been written by a housewife (by them).

4. He had been taught nothing by the experience of life.
Nothing had been taught (to) him by the experience of life.

Her children had to be taken care of by her.

.....take good care of.....の様にcareに形容詞が付いていれば、
Good care had to be taken of her children by her.
Her children had to be taken good care of by her.

の二つが可能だそうですが、careを単独でSにはできない様です。
