のような、主語と be の省略です。

省略というか、一部の接続詞では後にいきなり形容詞的なものが可能。

ここでは後の主語 these forms に対して

その直前に、 you will ..... and an automatic payroll deposit application.　とありますから、この文を読む限り、この payroll deposit application が省略されていると考えられます。

　Once (payroll deposit application is) completed, 　です。
