I was to be referred to by my first name

Millie has a daughter and a son, but they live on the opposite side of the country and aren’t able to visit now. Her daughter is 4, and when I saw her over the summer, she briefly referred to me as auntie. Millie pulled her daughter to the side and told her that I was not her auntie and that I was to be referred to by my first name.

前にも、referred to me as auntie とありますが、

refer to ～はおなじみ、「～に言及する、～に触れる、口に出す」
refer to A as B だと、
「A を B と呼ぶ、表す」

その受身が be referred to as ～です。

間接話法なので、人称と時制を考えて
「（あの人はおばあちゃんなんかじゃないし）
ファーストネームで呼んであげた方がいいよ」と言った。

英語的には受身ですが、日本語ではそのまま訳すと不自然です。
ただ、直訳的に頭の中で「呼ばれるべきだ」とするのもいいです。
be to は「～すべき」でいいかと。
