質問者さんの言う通り、not from は investigated にかかります。

だから

Aim:
Sup-epithelial connective tissue graft (SCTG)
and
de-epithelialized gingival graft (DGG) approaches

have been investigated
　　　(1) with a focus on post-operative morbidity
　　but
　　　(2) not from a clinical outcome standpoint.

という構造になりますね。

別の言い方をすると、

ABC and XYZ approaches
　　have been investigated with a focus on BBB
　but
　　have NOT been investigated from a clinical outcome standpoint.

という意味になります。
