主語はもちろん、文頭のMillennial caregivers。

主動詞は次のareで、almost twiceと続くので「ほぼ2倍である」と言っています。
つまり倍数比較をしており、それが直後にas~asの部分で出て来ます。
Millennial caregiversと比較されているのは、asの後ろのolder caregiver。

いろいろ入り組んでますが、まず、
be likely to~：～である可能性がある。
&
倍数比較の原級の構文は、
twice(or ~times) as 原級 as XX：XXの2倍(～倍)…である。
