ill now for these three weeks past...は、yesterdayにおいてnowであり、theseであるということですか。

for these three weeks past の中に yesterday も now も含まれていると思います。

普通、直接話法を間接話法に変えるときに

yesterday → the day before
this → that
という変換をする、と習うわけですが、
間接話法で yesterday/this のような単語が使われないわけではありません。

ひとつにはついさっき言ったばかりで場所も同じ、
時間的場所的隔たりもなければそのままです。

もうひとつ、今回のようなストーリーの場合、
ストーリー上で展開していることは
前の日は yesterday ですし、目の前にあるものは this/these です。

だから、この now もストーリー上の過去までにという
wife, who had been ill for ～ then ですし、
these もその展開している now までの「この３週間」です。

つまり、yesterday までに、というのが now であり、these three weeks ですが、
申し上げたように、yesterday 自体がストーリー上のものです。
