　・ Document が文頭にあるから命令文 （後ろに目的語があるので動詞と思われる）。
　・ the patient assessment information that ... 以下が目的語。
　・ ... that reflects A and guides B というつながり。
　・ the development and implementation of (1) and (2) という構造。

　患者の診断に関する情報を文書化せよ

　患者の現状を反映し、より進んだ治療や遂行を導くもの
