下記文章の英訳をお願いできませんか？

「私は確かにＣ－０９９６のレポートを受け取りました。
ご協力ありがとうございました。
私は同じ種類のback shellについて、もう一つ不具合連絡をしている。（Ｃ－０９９１）
それは2012/3/2にあなたに連絡済です。
不具合現品は2012/3/13に到着している筈です。

このレポートの発行も急いでください。

また、対策後、最初の製品をこちらに送る際、
包装箱かラベルにclaim No.　を記入してください。」

Ｃ－０９９６とは不具合品の管理ナンバーで
ここで言うレポートとは不具合品の対策書の事です。

何卒、宜しくお願いします。

I have really received a report of C-0996.

I appreciate your strong cooperation.
As for the same kind of back shell, C-0991, I have contacted you for another defect.
I have already finished contacting on the 2nd, 3, 2012.
I’m sure you have already received that defective item on the 20th, 3, 2012.

Please issue a report for this item as soon as possible.

Again, when you send the first item to us after your great work,
please mark its claim number on its package or its label up clearly.

C-0996 means its management number of a defective item.
A report we are describing here means a provision manual for defects.
