いまお送りさせていただいているファイルのフォームに記入し返信をおねがいします

It would be appreciate if you could fill out and send the forum i have attached.

もう少し正しく修正していただけませんでしょうか。

三番です、追加情報というか知っておけばそれらしくなると言うととても短い単語「duly」について、、、、、、、

意味は「正式に、キチンと、正当に、間違いなく」ってな意味ですが、下記の通りある程度格式張った書面には頻繁に使われております。

これを使って、もう一度書き直すと、こんな具合に、、、、、、

これ殆ど決まり文句ですね。

両者とも、返送期限を明示しておいた方が良いでしょう、例えば、、、

みたいに、、、、、、、、、

もしレターにその内容を書いていて、フォームが別にある時は。

Please find the enclosed form.と前置きしてもいいかもしれません。

非常に丁寧な英語ですね。大きなミスはappreciateのところぐらいです。あとはそのままでも十分通用すると思います。
